The Human Support Group - Plymouth (HSG) Reable program aims to support individuals in readjusting to living at home after a period of up to 6 weeks, enabling them to live independently with as little support as necessary. HSG Reable helps individuals leaving hospital or respite care to regain their independence and stay safe during their recovery. The program assists individuals in setting personal goals for their care and creates individually tailored reablement plans to reduce the amount of support needed over time. HSG Reable operates 365 days a year, 7 days a week from 7am-11pm, and accepts referrals from Local Authorities for individuals with the potential to improve and regain their independence.
