Coordinate My Care - Care Plan

Coordinate My Care is a service that facilitates urgent care for patients by utilizing an online Advance Care Planning questionnaire called myCMC. The completed myCMC is then used by a doctor or nurse familiar with the patient to create the Coordinate My Care (CMC) plan, which includes the patient's diagnosis, medical details, resuscitation status, medications, and recommendations for urgent care services in the event of an emergency. Once approved, the plan is immediately accessible to all urgent care services, ensuring that everyone involved in the patient's care is informed and coordinated.
